Membership Fees

 

Monthly Fees

  • Adult $110

  • Child (age 2-20 years, joining with an adult) $60

  • Child (age 2-20 years, joining without an adult) $86

  • Infant (age 0-2 years) $140

*Family maximum is $350/month, so if you have enough family members that the cost would be over $350, you only pay $350. If you have fewer family members joining then the cost per month is the sum of the individual amounts (includes only one household).

Enrollment fees are due at the onset of joining the practice, they do not recur monthly.
